Case Management services focus on assisting those having difficulty securing affordable housing and a financial means to support themselves.
Case Management Services include:
- Support and advocacy to homeless individuals and families.
- Assessment and goal planning.
- Assistance in securing housing and financial support.
- Community outreach services.
- Several months of follow-up services.
